Appointment Information

Please use the following format for each session:
Date – Time – Client’s Name – Session Number – Attendance Status (if providing this/last week’s details) – Referral Type – Any notes

For example:
01/01/2019 – 9.30am – Gertrude – 2 – A – £

For the Attendance Status, use the following codes:
A=Attended
DNA=Did not attend
C=Cancelled with at least 24 hours notice
CX=Cancelled with less than 24 hours notice

Please use the following codes for the referral type:
NHS=NHS client
£=Commissioned client
FIP-Parenting 2000 FIP family
